Canvas allows end users the ability to print an integrated care plan from the patient summary. The care plans in Canvas detail the patient's goals, medications, allergies, conditions, and future appointments. It also displays patient details and staff members assigned to the patients care team. 

FAQ

Q: Why can't I select the Print Integrated Care Plan option in the triple dot menu?

A: Goals must be documented in order to enable the Print Integrated Care Plan option.

Step By Step

Requirements

  • Care Team and Goals features must be enabled in your Canvas environment. If you are unsure if these features are enabled please contact Canvas Support.

  • Once the Care Team feature is enabled, care team roles must also be configured in order to assign staff members to a role. Only members with roles that fall under the clinical domain will display in the team member drop down menu.

  • At least one goal command has to be documented in the patient's chart to successfully use the print integrated care plan option. 

Information in an Integrated Care Plan

  • Patient Details from Registration page, including:

    • First Middle Last Name

    • DOB

  • Organization Details

    • Name

    • Address

    • Phone

    • Fax

  • Care Team members assigned in the Registration page 

    • Name

    • Role (Care team lead at the top)

  • Goals from from Patient Summary

    • Name

    • Due Date

    • Current Status

    • Progress/Barriers

  •  Medications from Patient Summary

    • Name

    • Sig

  • Allergies from Patient Summary

    • Name

    • Severity

    • Reaction

  • Conditions from Patient Summary

    • Name

    • ICD 10 code

  • Upcoming Appointments

    • Appointment requests triggered by follow up command display the follow text:

      • Come back for "visit type" around "date requested" for "reason for visit"

    • Appointments scheduled from appointment notes or schedule display the following information:

      • Provider Name 

      • Appointment Type

      • Reason for visit

  • Patient Signature Section that includes:

    • Field for signature

    • Date

    • Field for printed name (If not signed by patient)

  • Footer Disclaimer that reads

    • If you have any concerns or questions about your visit, please call "organization's phone".

Print an Integrated Care Plan

  • Navigate to a patient's chart where at least one goal command has been documented 

  • Locate and open the triple dot menu in the patient summary (next to the patient name)

  • Locate and select the Print Integrated Care Plan option

  • Change print settings as needed in the print modal 

  • Select Print
