Introduction

Social determinants of health are the economic and social conditions that influence an individual health, functioning, and quality-of-life outcomes and risks. Examples include the following:

  • Where people are born

  • Where people live

  • Their age,

  • Where they learn

  • Where they work

  • Where they play

  • Where they worship

In Canvas the social determinants appear in the patient summary and are determined by answers to corresponding questionnaires. The following questionnaires can be made available and automatically populate the social determinants. Please contact your Implementation team or Canvas Support if you need to add them to your environment(s).

  • Stress

  • Tobacco (linked to the Tobacco: Screening and Cessation Protocol)

  • Nutrition

  • Financial Strain

  • Health Confidence

  • Self Health

  • HARK (Intimate Partner Violence)

  • Audit-C (Alcohol)

FAQ

Q: What if none of the questionnaires available are the one I need?

A: Questionnaires are can be modified to fit the needs of each clinic. Please reach out to Canvas Support for further information.

Step by Step

Navigate to an open note of the desired patient's chart

  • Type "questionnaire" into a blank line within the note

  • Select the command by hitting Enter on the keyboard

  • Select the desired questionnaire from the available optionsĀ 

  • Answer the questions presented in the questionnaire

  • Navigate through the questions using Tab and the arrow keys on the keyboard

  • Select RECORD

  • Update questionnaire answers by repeating the steps above

NOTE: The most recently documented results will show up under social determinants in the patient summary.
