Preparing Provider Profiles for Enrollment

Before you can enroll a provider for prescribing or lab ordering, their Canvas staff profile needs to have all the right information in place. This article walks you through everything so there are no surprises when you're ready to submit.

Staff Profile Required Fields

Before starting enrollment, make sure these fields are complete in Settings > Staff for the provider.

Required for all providers

  • First Name: Legal name matching medical license

  • Last Name: Legal name matching medical license

  • NPI Number: 10-digit identifier

  • Primary Practice Location: Select from available locations

  • Work Email: Used for enrollment communications

  • Work Phone: Numbers only, no special characters or spaces — 5551234567

  • Work Fax: Numbers only, no special characters or spaces — 5551234567

  • Work Address: Complete street address, city, state, and ZIP code. Set the Use field to Work and keep the Type set to Both.

Required Documents for Surescripts

You'll need to gather these documents before submitting the provider enrollment form.

  • PersonaTrust Certificate: The provider receives this via email after completing identity proofing. Save the PDF for enrollment submission.

  • State License: A clear copy of at least one state medical license

  • DEA License: Only required for controlled substance prescribing

Licensing Requirements

For all prescribing providers

  • Medical License: Required for eRx and EPCS

For controlled substance prescribing only

  • DEA License: Required for EPCS and Schedule II-V prescriptions

How to Add Staff Licenses

Providers need valid licenses on file before they can prescribe or order labs. Here's how to add them.

  1. In the provider's staff profile, scroll to the Staff License section

  2. Click + Add another Staff license

  3. Complete the following fields

    • Issuing Authority Long Name: Only required if selecting Other for license type. Leave blank otherwise.

    • Issuing Authority Short Name: Leave blank

    • Issuing Authority URL: URL to the issuing authority's website. Optional.

    • License or Certification Identifier: Enter the license number

    • Issuance Date: Date the license was issued in MM-DD-YYYY format

    • Expiration Date: Date the license expires in MM-DD-YYYY format

    • License Type: State License, DEA, CLIA, PTAN, Taxonomy, SPI, or Other

    • Primary: For State and DEA licenses only. The primary license appears on external documents like prescriptions.

    • State: For State License type only. The state that issued the license.

  4. Click Save

  5. Repeat for each additional license

Good to know — You can add multiple licenses, but only one per type can be marked as primary. If no license is marked as primary, the first entered license will appear on external documents.

Q: Can I start enrollment before identity proofing is complete?

A: No. Both Surescripts and DrFirst require identity proofing before they'll process enrollment. Starting without it will result in rejection and delays.

Q: What if the provider doesn't have a DEA number?

A: No problem—DEA numbers are only required for controlled substance prescribing. Providers can prescribe non-controlled medications without one and add a DEA number later if needed.

Q: Do all these fields need to be complete before I submit enrollment?

A: Yes. The Provider Enrollment Dashboard validates all requirements and only allows enrollment when everything passes. This prevents rejection and delays down the line.

Licensing and formatting

Q: Why do phone and fax numbers need that specific format?

A: External systems like Surescripts and Health Gorilla expect pure digit strings. Formatting characters like dashes and parentheses cause transmission errors.

Q: What happens if I don't mark a license as primary?

A: For State and DEA licenses, the first entered license will appear on external documents like prescriptions. If you want a different one to display, mark it as primary.

Identity proofing

Q: How long does identity proofing take?

A: Appointments are about 30 minutes. The provider needs a government-issued ID and should have their Experian credit report handy to help answer verification questions.
