Introduction

When sending a fax containing protected health information, Canvas provides users with the ability to add a fax cover sheet. Fax cover sheets sent from Canvas include standard HIPAA and privacy statements.

Tips & Tricks

• If you are not presented with the ability to add a fax cover sheet ensure the FAX_COVER_SHEET setting is enabled within Config

• Faxes generated from the refer command or image command do not have the option to include the fax cover sheet

• Practice location details are auto-populated into the fax cover sheet at the bottom left corner, this information is based on input in the fax module. To change the location within the fax modal, users can make changes to location on a note or within the letter.

• Faxes are able to be sent without a fax coversheet when include cover sheet is not checked.

Step By Step 

Adding a Fax Cover sheet

Cover sheets can be added to faxes sent from notes and letters on a patient's chart or through data integration.  

• Beginning with a note, a letter, or a document in data integration, select fax

• A fax module is populated allowing you to add a fax cover sheet

• Select the check mark next to include cover sheet to expand the module

• Complete the required fields and any optional fields within the module

  • To (required):  Intended receiver of the fax

    • Fax Number (required): Destination fax number

    • Practice Location (required): Sending practice location (will be defaulted based on note type if sent from note or letter)

    • Subject (required): Reason for the fax

    • Comments (optional): Any comments to give better understanding for the reason of the fax

• Select Next to see a preview of the document including fax cover sheet

• After entering the required information and selecting next, you are now presented with a preview of the fax before sending

• Review the cover sheet and intended fax 

• If you decide to no longer send the fax select cancel

• If you need to make changes to the cover sheet, select back

• If the fax meets all expectations, select send
