Command | Physical Exam

Last updated: October 6, 2025

The Physical Exam command is available to document an examination that has been performed. This documentation can be concise or all-inclusive.

FAQ:

Q: I made a mistake, how do I remove this command?

A: Commands can be entered in error, but not deleted.  Please review the Entering a Command in Error document for directions.

Q: Do I have to use a general exam template for patients with an acute concern?

A: Exam templates are available to all users within the practice. For personalized content, you have two options:

  1. Automations: Create automations to prefill physical exam commands with your preferred specifications and placeholder text. Access this through Settings > Automations (under the Practice heading).

  2. Text Expansion Tools: Use third-party snippet-saving tools like TextExpander or Blaze to create personalized shortcuts for frequently used exam findings and phrases.

Both approaches allow you to customize your workflow while maintaining the shared template structure for your practice.

Q: Can I create my own templates that do not interfere with other providers in my practice?

A: Exam templates are available to all users within the practice.  Automations are recommended to assist with customizing personal workflow.

Q: I've toggled a system off, but it's still displayed in the note.  How can I get rid of it?

A: Toggled systems within the physical exam command are still visible in the digital representation of the chart where it is clear to see the off toggle is present.   Printed or faxed representations of the note will not display the removed systems.

Step by Step

This process begins with an open note of the patient chart.

  • Type "physical exam" into an open line within the note.

  • Select the command by hitting Enter on the keyboard or by clicking the option.

  • Select the type of examination that has been performed from the drop-down options

NOTE: The examinations can be customized to fit the needs of the practice.

  • Navigate through the remaining fields by selecting Tab between each field

  • To remove a system that was not evaluated at this visit, un-select the toggle next to the system name.
